When assessing a client and preparing a care plan, our rehabilitation specialists are reviewing all components of client well being. This includes living arrangements & transportation, occupation & income status, psychosocial history (social history, education, employment & transferable skills, recreation & leisure activities, vocational rehabilitation), medical status (physical & psychological).
REGISTERED SUPPORT WORKERS
Implement rehabilitation plans in consultation and direction of speech
language pathologist, occupational, therapists, physiotherapists, psychologists etc.
PERSONAL SUPPORT WORKERS
Implement personal support care plans under the direction of health care
professionals, to enable clients to live safely with the degree of independence possible in their circumstances.
ATTENDANT CARE WORKER
Providing basic supervision to client, the attendant may
supervise the client throughout the day and assist with ADL's.
ASSISTIVE DEVICES
Adaptive, and rehabilitative devices for people with impairments to help perform activities of daily living independently, or with assistance.

Case Management is a collaborative process of assessment, planning, facilitation, care coordination, evaluation and advocacy for options and services, to meet the client's and client family's comprehensive health needs.
Case manager will assist with identifying client service needs and will help identify appropriate providers and facilities throughout the continuum of services, while ensuring that available resources are being used in a timely and cost-effective manner. Clinical
Case Manager will assist with gathering and coordinating the variety of medical, financial, social and personal services required.
Collection and review of all information relevant to the clients' situations including the health and social factors that may impact their needs.
Identification of priorities and clients' needs that Case Management can address.
Assessment of clients' physical, psychosocial, emotional, spiritual and cultural factors (i.e. body, mind and spirit).
Integration of subjective and objective information from multiple sources including medical records, clients, social networks and the various stakeholders.
Prioritization of service needs with clients' input
Set short- and long-term goals in collaboration with clients and their social networks.
Communicate the agreed-upon Case Management plan to the identified stakeholders.
Maintenance of the oversight of clients' Case Management plan.
Motivation of stakeholders to sustain and maintain support for clients' goals.
Collection and provision of feedback on services provided by stakeholders involved in the clients' plan.'
Modification of the Case Management plan and the use of service providers as necessary.
